**When you complete the survey, please fill out the attached form and take it to the office.
The week of November 5, SFPS will begin distribution and collection of Parent Academy Surveys. The purpose of this survey is to collect information which will guide the District in determining the types of courses to offer our parents as well as their preferred locations and times for courses.
The District will be holding a district-wide raffle, and students who return their parent survey will receive a raffle ticket for the district-wide raffle. The grand prizes will be i-Pads (2-secondary, 2=elementary) and there will be additional prizes as well. A raffle ticket will be provided to each child who returns their paper survey or completes the survey online.
The date for the elementary schools drawing will be determined soon, but all elementary school raffle tickets will be collected on Wednesday, November 14th.
Along with the paper surveys, parents will also receive a Parent Academy flier for each child to take home, a letter from the Superintendent announcing the survey.
